The Application Process

The student tracking form (Enquiry Form) needs to be filled up and submitted online in the first instance. Once the administrator confirms your eligibility to enroll for the M.Ch Orthopaedics , we will send you a confirmation by email.

Once you receive this confirmation the Definitive Student Form (Registration Form) should be downloaded and printed and completed. They should now be dispatched to the M.Ch office with the necessary photographs, certificates and your curriculum vitae. Payment must accompany this full length application form for admission.
